COVID-19 vaccines for children ages 6 through 17 years old –
We are providing vaccine appointments at our outpatient pharmacies

How to register for an appointment:

By Phone:

Call 954-276-4340 Monday through Friday 8 am to 5 pm to register for an appointment or login to MyChart as your child proxy/caregiver.

MyChart:

  • Adolescents who have an existing MyChart account or caregiver with proxy access (caregiver access) to a teen's MyChart account, can login to schedule an appointment I
  • f you need proxy access to an adolescent's account to make the appointment visit our proxy account information page for instructions on how to request access
  • If you or your adolescent is using the website version of MyChart to schedule and appointment, after logging in, choose "Visits" and then "Schedule an Appointment"
  • If you or your adolescent is using the mobile app version of MyChart, after logging in, choose "Appointments" and then "Schedule an Appointment" If you are unfamiliar with MyChart we have created a step-by-step guide on how to schedule a vaccine appointment
  • If your adolescent doesn't have a MyChart account please schedule the appointment by phone.

Second dose appointment

You can schedule your second dose appointment as soon as you receive your first dose. We recommend using MyChart for scheduling; however, you can also call 954-276-4340 Monday-Friday 8 am to 5 pm. Remember to bring your vaccine card when you return for your second appointment.

What should I expect when I arrive for my appointment?

  • Vaccines will be provided only by appointment.
  • Only one parent/guardian is allowed with the patient, and parent must be present at the time of the vaccine administration.
  • Please do not bring younger siblings with you to the vaccine site.
  • No pets allowed. Service animals, with documentation, allowed.
  • Wear a shirt or top with the upper arm accessible.
  • You will be given a vaccine fact sheet and asked a series of questions including any allergies.
  • Once you receive your vaccine, you will be observed for thirty minutes. You can use the time to schedule your follow up appointment for the second dose.
  • When you get your first dose, you will get a vaccination card to show when you return for additional doses if needed. Remember to bring your card when you return. If you lose your vaccine card you can find your vaccination record on MyChart.

What should I bring to my vaccination appointment?

  • Parent must bring proof of age.
    • Birth certificate
    • Passport
    • State issued ID card, Driver’s Permit or Driver’s License
  • If you are coming for your second or third dose bring the vaccine card provided to you on your first dose appointment. If you lose your vaccine card you can find your vaccination record on MyChart.

PLEASE NOTE: No walk-ins will be accepted at the vaccination site. On-site registration is not available. You must register for an appointment before coming to the site by calling 954-276-4340.

Is there a cost for the vaccines?

Vaccines are provided at no cost to recipients and insurance is not required. However we will request consent to verify insurance coverage; no fees will be billed to the patient.

Joe DiMaggio Children's Hospital is committed to administering the COVID-19 vaccine to individuals regardless of the vaccine recipient’s ability to pay COVID-19 vaccine administration fees. The state of Florida has developed a pre-registration system to schedule COVID-19 vaccinations. For more information visit myvaccine.fl.gov
