New Patient Appointment Scheduling
After scheduling your first appointment, please download and complete the following forms and bring them with you to your appointment. This will save your time during registration.
To download the following forms, you must have Adobe Reader installed on your computer.
If your appointment is for diabetes care, please also bring the following:
- Forms from your last two visits with your child's endocrinologist, or other doctor managing your child's diabetes
- Growth chart
- Previous two labs
- Your child's Meter and Blood Glucose Logs
If your appointment is regarding your child's growth, please also bring the following:
- Growth chart from referring doctor
- Last visit with referring doctor
- Lab results
- X-ray reports
If your appointment is not related to diabetes or growth, please also bring the following:
- Growth chart
- Last visit from the referring doctor
- Any labs ordered
